Prob-Fluid

Research Question

Does a protocolized dialysis prescription regimen result in a lower net fluid balance than usual care?

Patient Type

Renal

INTERVENTION Type

Nonpharmacologic

GROUP A

Dialysis with protocolized fluid balance targets

GROUP B

Usual care

INCLUSION CRITERIA

· Age ≥ 18 admitted to the ICU

·Decision to initiate KRT (Kidney Replacement Therapy) within 12 hours or  KRT in progress for less than 48 hours

· Acute kidney injury during current hospitalization defined as any of the following:

o  Increase in serum creatinine by 27 μmol/L or more within 48 hours,

o  Or an increase in serum creatinine to 1.5 times baseline or more

within the last 7 days,

o   Or a urine output less than 0.5 mL/kg/h for 6 hours.

INCLUSION TIME LIMIT

48h post initiation of dialysis

EXCLUSION CRITERIA

·  Lack of commitment to maintain kidney support at the time of screening, or probable

transition to comfort care within 48 hours according to the treating physician

·        Probable discharge from the ICU within the next 48 hours according to treating

physician

· Severe burn injury (>10% body surface area)

·   Severe abnormalities of serum sodium (>155 or < 120 mmol/L)

· Important ongoing fluid losses are present and are expected to require continued

maintenance IV fluids during the next 48 hours

  • The clinician believes that the proposed intervention is contraindicated
